Hallucinations
Hallucinations in AI refer to instances where an artificial intelligence system generates information that sounds confident and plausible but is factually incorrect, fabricated, or unsupported by its training data or source material. The term applies most commonly to large language models, which can produce false statements, invented citations, or fictional details while maintaining the same tone and structure as accurate responses. Hallucinations matter because they erode trust in AI outputs and can lead to costly mistakes. A hallucinating model might cite a regulation that doesn't exist, generate a patient summary with incorrect medical history, or fabricate financial data in an analyst report. In high-stakes environments, acting on hallucinated information can create legal liability, compliance violations, or safety risks. Hallucinations happen for several reasons. Language models are trained to predict the most likely next word in a sequence, not to verify factual accuracy. They don't have a built-in sense of what is true — they have a statistical model of what sounds right. When a model encounters a question outside its training data or at the boundary of its knowledge, it tends to fill in gaps rather than say it doesn't know. Retrieval-augmented generation, grounding techniques, and confidence scoring are common approaches to reducing hallucination rates, though none eliminate the problem entirely. For enterprises deploying AI across departments, hallucination risk needs to be managed as part of the broader AI governance strategy. This means testing models for hallucination rates before deployment, implementing human review for high-stakes outputs, and monitoring production systems for factual drift over time.
